Medicolegal Case Documentation 

Course Overview

Medicolegal Case Documentation is a practical, competency-based Skills Lab lesson designed to help undergraduate MBBS students develop the knowledge, skills and professional attitude required for accurate documentation of medicolegal cases.

The lesson moves beyond theoretical learning and provides students with an opportunity to observe, practise and document a simulated medicolegal case using structured case records and documentation forms.

Students are introduced to the complete workflow of medicolegal documentation:

IDENTIFY → HISTORY → EXAMINE → DESCRIBE → DIAGRAM → INVESTIGATE → TREAT → REFER/DISPOSE → AUTHENTICATE

The session emphasizes the fundamental principle that students must document the patient's statements separately from their own clinical findings, describe injuries objectively, avoid unsupported assumptions, maintain confidentiality and preserve the integrity of the medical record.

LEARNING APPROACH

The lesson uses a Skills Lab and simulation-based teaching approach, incorporating:

Students are encouraged to actively complete documentation rather than simply observe the instructor.

LESSON COMPONENTS

1. Introduction to Medicolegal Documentation

Students understand the purpose and importance of accurate medicolegal documentation and the doctor's responsibility to maintain an objective clinical record.

2. Patient Identification

Students practise recording essential patient identification details, date, time and source of information accurately.

3. Alleged History

Students learn to document the patient's account of an incident objectively and distinguish the patient's statement from the doctor's observations.

4. Clinical Examination

Students practise systematic examination and documentation of relevant clinical findings.

5. Objective Injury Description

Students learn to document injuries using appropriate anatomical terminology and relevant features such as:

• Site
• Side
• Type
• Size
• Shape
• Orientation
• Other observable characteristics

6. Body Diagram Documentation

Students practise accurately mapping injuries onto a body diagram and correlating injury numbers with written descriptions.

7. Investigations and Treatment

Students learn to record relevant investigations, findings, treatment, procedures, referrals and disposition.

8. Consent, Refusal and Confidentiality

Students are introduced to appropriate documentation of consent/refusal and the importance of confidentiality and secure handling of medicolegal records.

9. Authentication and Record Integrity

Students learn the importance of appropriate date, time, identification and signature, as well as appropriate correction of documentation errors without compromising the integrity of the original record.

10. Complete Simulated MLC Documentation

Students integrate all components by completing a simulated medicolegal case from initial identification through final authentication.
